Prescription Drug Cards for Protector Plus & Apex
Policy Holders
This week, new prescription drug cards
were mailed to your Protector Plus and Apex policyholders
that have a current Caremark prescription drug card.
Clients should discard their current drug card and replace
it with the new card immediately, as pharmacies will
no longer be able to process prescription drug claims
when the old drug card is used.
The new drug card comes with a revised prescription
drug benefit, as described in earlier communications:
- $50 per person annual deductible, then:
- $15 Copay for Generics
- $30 Copay for Formulary Drugs after deductible
- $50 Copay for Non-Formulary Drugs after deductible
- 25% Copay for Specialty Drugs after deductible
- No coverage for Mental Nervous Medications*
- Oral Contraceptives covered only in Illinois, Arkansas,
and Missouri
*Does not apply to policyholders in Arizona, policyholders
with effective dates prior to 4/1/07 in Illinois, or
policyholders with effective dates prior to 8/17/09
in Arkansas.
