E-Applications * New for United Security Life and Health Insurance Company

After quoting any of our Major Medical plans, you will now see an option to use our e-application. When you choose this option, the application will automatically populate any of the information you filled out during the quoting process. From there, you simply answer the questions and submit the form electronically. You will receive an immediate e-mail confirmation that USL&H has received the application and will begin the underwriting process. In order to complete the application, you MUST fax a copy of the down payment check and signed application to (708) 475-6121, Attention: Data Entry, and mail the original documents to our home office. (Note – We must have your updated e-mail address on file in order to send you the confirmation).

Be sure to check out our new major medical applications that allow you to apply for any one of our major medical plans from the same application. Feel free to download and print copies immediately. Please destroy all of your old applications. Underwriting will not accept applications submitted on the old forms after 9/1/07.
