On January 1st, the Protector Plus PPO
Plan is being reintroduced in the states of Arizona,
Arkansas, Illinois and Nebraska!
The Protector Plus - known for its flexible plan design
- will complement our current portfolio of Major Medical
products (Unlimited Access, HealthSelect PPO, Preferred
Value HDHP & Healthy Savings HSA-Qualified).
A few things have changed since you last sold a Protector
Plus policy, so make sure to take a look at our new
brochure. However, the basic plan structure remains
the same:
- Deductible Options ($500, $1,000, $1,500, $2,500,
$5,000 & $10,000)
- Coinsurance Options (80/20 to $10,000 or 80/20
to $5,000)
- Lifetime Maximum Options ($2 million or $5 million)
- Common Accident Benefit
- Prescription Drug Coverage
- Optional Benefits (Prescription Drug Card, Office
Visit Copays, Accident Benefit, Dental Benefit, Term
Life & More!)
Here's the best part ... you can begin quoting Protector
Plus at our website now! The plan can only be quoted
with an effective date of 01/01/10 and later.
As a result of these changes, USL&H has also updated
its online rater, sales brochures, agent guidelines
and applications. The USL&H desktop rating software
will be updated with the new rates in early January.

New Business Rates Available For 01/01/10
Please review the following. Your new business applications
will receive 10/01/09 rates or 01/01/10 rates based
on the following criteria:
- USL&H will honor the 10/01/09 rates on all
applications received prior to December 17th, regardless
of requested effective date.
- The only other policies that will receive 10/01/09
rates are applications that meet all of the following
criteria:
- received at USL&H between 12/17-12/28
- with a requested effective date of 12/28 or
earlier
- where USL&H receives ALL necessary underwriting
information (complete app, payment, PHI and records)
prior to 01/01/10.
- All other issued policies will be subject to the
01/01/10 new business rates.
