USL E-signature now available

After quoting any of the United Security Major Medical plans, you will now see an option to use the e-application. When you choose this option, the application will automatically populate any of the information you filled out during the quoting process. From there, you simply answer the questions and submit the form electronically. You will receive an immediate e-mail confirmation that USL&H has received the application and will begin the underwriting process.

Now, you can complete the e-application without faxing in ANY information, thanks to our new PAC down payment option. When an applicant selects PAC as a payment option, you only need to fill in a few pieces of information (name of depositor, name of bank and city/state where it is located, account and routing numbers), which can be obtained from the depositor's check. There is a field for you to submit this information electronically so that we can begin processing the application right away. It is important to note that when an applicant selects PAC as their payment option, the application fee and initial premium amount will be drafted from their account immediately - NOT at time of issue.
