The "Advantage Care" HSA Qualified Plan has been a HUGE SUCCESS! (Brochures and applications are available below) The Advantage Care plan offers four (4) different deductible options for individuals and families:

Individual: $1500, $2500, $3750 and $5000
Family: $2500, $5000, $7500 and $10,000

Co-insurance: 100% !!

In addition to the above choices, our "Advantage Care" plan also offers

Preventative care up to $125 per year per insured (subject to deductible)
Dental Care Benefit: No waiting period: No deductible
Optional Full Maternity Benefit : as any other illness (Not available to residents of Indiana).
